PWS Lenses (EyeGlasses and Sunglasses) Prescription

Overview

Custom Module (Extension) for adding lenses (eyeglasses and sunglasses) prescription (Sphere, Cylinder, Axis, Near/Add, PD) to a product.

The customer can enter this prescription in a special form on the product view page. The prescription is saved as a Magento custom product option and is visible on the order email, customer order details and on Magento admin order details. The prescription is also saved in the customer account, the customer can view/modify his prescription.

Features

1. comment field for prescription
2. switch between single pd or pd for left/right eye
3. the Prescription form options Sphere, Cylinder, Axis, Add, PD can have extra prices; eg: sphere +6 costs +15 USD
4. Extensive configuration:
   - Glasses Usage options are configurable, one can add as many options, each option can have an extra price
   - Prescription fields are configurable: hide/show near/add option, show single PD or PD for left and right eye, show/hide comment form
   
5. The prescription form is saved and can be viewed/modified by the customer from his My Account section
6. The prescription form on the product page is automatically filled in (if there is a prescription saved) when the customer logs in
7. The prescription form is saved as a custom product options and it's displayed on cart/order/invoice/shipping/credit nota pages, emails or pdfs.
